This is really testing one's memory,isn't it? From memory:
- support for SCSI disks arrived around 5.3
- there's no support for SCSI in V4.0
- the savesets are called VMS040.A REQUIRED. OPTIONAL. LIBRARY. ,
right?
Which one did you restore to build a bootable disk?

Inicentally, why don't you use SIMH's VAX 11/780 to install VMS 4.0
with?
