I thought I missed the original question. On reply to the comp.os.vms
branch I noticed it was posted in comp.sys.dec first, bit that this
was the whole question!

- The record length words are word aligned, so there may be round-up
bytes (nice place to store a secret message :-)
- There is an MSB variable lenght flavor with the lenght word bytes
swapped. Created to support certain CD's. Never used really.
- Guess you should take care of VFC records as well, and notably
decide whether to interpret PRN/VFC data or pass binary

I have something like that somewhere as well, but can not readily
find it.
Mine was written by Tony Boyer (Bower?) a guy from the UK who worked
for Digital Education Services back in the 80's. Runoff, A4. I'll try
to find it.

McCoy, Digital Press 1990.
No RMS in there. But McCoy was tasked to write up RMS internals and
did so (DEC Document) but only made it to a single, albeit critical,
chapter describing indexed file with compression and all. I have that,
but only in hard-copy, pre-edits, pre-publish. I'll consider scanning
that in but it is a 'light' printout.
