Hoff has a blog about building and using SimH on OS X.

I recently built SimH (3.8.1) on Ubuntu Linux with network support as a
way to check something on an ancient version of VMS. I haven't tried a
build of 3.8.1 on OSX but Hoff's blog looks straightforward enough. The
Ubuntu build presented me with no problems save for trying to understand
a configuration (boot, not build) item to get one feature functioning.
