The default username/password for the Ricoh printers has changed between 
some models.

Some that I've seen:

If only a password is asked, enter 'ricoh'.

If a username/password is asked, enter 'admin' at the login, and a <CR> at 
the password prompt.  The MP4500 I just tried was like this.
