If you have the right software I would think you ought to be able to 
lift the image right out of the PDF.

I couldn't do that, but I ran book reader, went to the EDT reference 
manual (in archived manuals) on OVMSDOC071 CD (presumably new versions 
exist), went to that diagram, then print-to-file.  Read the resulting 
postscript file in Ghostscript, converted to an image file, and loaded 
right into my MSWORD document (properly attributed, of course).
