What account are you using?

$ DEFINE/SYSTEM/EXEC TCPIP$SSH_SERVER_DEBUG DEBUG

and start...

Take a look at SYS$SYSDEVICE:[TCPIP$SSH]TCPIP$SSH_RUN.LOG
