Our first PDP applications used the term "Unexpected error" as part of 
the standard error routines.  It meant what it said.  We didn't expect a 
record to go missing just after we (thought we had) read it for example.

In practice such errors were normally encountered as a result of program 
bugs, but they also encompassed things like a disk going offline.
