I also have an Alphaserver 1000A and I have put up to five KZPBA-CX
adapters in it. I have also a variety of BA356 boxes with either wide
personality modules or Ultra personality modules. I have never seen
the problem you have.
However, I saw that the 1000A boots just fine (from an internal disk)
when no external cable is connected to any of the KZPBA-CX cards. So
if you disconnect the SCSI cables you should be able to locate the
problem to either the 1000A or the BA356.
