Lack of backups, or perhaps lack of testing that backups can be restored
in a reasonable time frame?
A forum I used to read went down for several days a few years ago, all as
the result of MySQL running out of disk space. The guy running the forum
was technically knowledgeable, but he simply didn't have the in depth
experience as a DBA to recover. Fortunately a forum member who did have
that in depth experience was available to help, and only a small amount
of data was lost.
Oh, the guy didn't have any tape backups because he "didn't trust tape
backups".
