Three Mile island was solely a case of inadequate education and training.  A 
valve had 600 PSI on one side, and 200 PSI on the other side, and the operators 
thought it was closed.  It was open.  They never tried to close it.

My son is an operator at a nuclear power station.  This is included in their 
training, along with many other things.  Accidents can happen, but inadequate 
training is not an accident.
