"Opcode reserved to DEC" is (was) correct.  It originally meant machine
code on a VAX that didn't translate to any valid VAX instruction. It
didn't matter whether the VAX was running VMS, Unix or anything else, the
instruction still does not mean anything to the VAX.  Since then
equivalents have been created for Alphas and Itanics.

The usual cause is attempting to execute data or corrupted instructions
or so forth.
