There were two questions, one about the architecture and one about
the implementation. I wanted to finally answer the former.
As someone else noted, VAX doesn't allow for all 32 bits in the
physical address, even at the architectural level.
Also, the details of the five levels of tables, and the ways to
use only three or four when addresses aren't so big, are all
described.
