ConsoleWorks from TDI and ConsoleManager from CA (I think...) do this and a lot more which makes them expensive.   Console Manager probably still only runs on VMS, where ConsoleWorks does well on other O/S.  One way to help justify the expense is to connect to any / all other devices that have serial ports (even if they are accessed via ILO or other TELNET/SSH) for out of band management as a back door to error discovery and recovery.
