After rereading this last sentence, I'll say again that you've
made things far too complicated.  *If* your desire is to make
the quorum disk mandatory (why in the world would you want to do that???), then give the quorum disk one vote, the two nodes each
zero votes, set expected_votes to one and be done with it!

Do be aware that if you quroum disk starts to fail, you're going
to have a hell of a time replacing it...  And since it can't be
shadowed, you're at greater risk than ever.
