But even systems which are firmly behind a firewall can have networking 
issues.  I clearly remember a bit of application software on VMS falling 
over when another department expanded the size of an NFS served disk.

FWIW that example was in a bit of disk free space monitoring software 
and was an easily fixed integer overflow because it was developed 
internally and therefore we had the sources.  I have also come across 
systems where sources are out of data or incomplete.
