In the case of ext2 and friends I don't have a full answer but they have
always decoupled the filesystem block size from the underlying physical
block size and having that support appears to have helped them here.

As for other filesystems the effort involved appears to vary enormously
by filesystem but I have not explored any specific low level details.

Having to explore 4096 byte sectors at a very low level is not something
I have (yet) had to do. :-)
