I was finally able to finish off my custom Apache authentication module today (had been dragged in a different direction for the past 4-weeks).

Can do cookie authentication
Can do username+password authentication
Can do group-access restrictions
Can command a browser to raise a username+password dialog
Can command a browser to store a cookie
