It's been my experience that different organizations and differing 
teams (unsurprisingly) have differing abilities to contend with 
managing chaos and with changes in scope and scale.  Most organizations 
tend to operate near the edge of what they can cope with, in terms of 
scale and complexity and chaos.    Some organizations can react and 
adapt.  Others...  Well, throwing staff at a problem — flailing — isn't 
a viable solution for many messes.  As that book mentions.  There are, 
however, ways to incorporate and to use additional folks when working 
at and increasing scale, and there are organizations and managers that 
can successfully manage projects at scale.
