How about the number of .MAI files in a directory?  Maybe hitting the cache limits?  I'd look for count problem first.  Since you have "compressed" the files to minimize headers, the next thing is directory entries I would think.

Are there excessive file create/delete operations?  Look specifically for deletes in a large directory with many .MAI files.  Entry shuffling between directory file blocks?
