Since the cleanup will likely take a while, rename the top level directory to a temp name and recreate the main. This will "rename" the files out of the way quickly. Rename the MAIL.MAI back to the original directory and continue. Note that this will prevent MAIL from seeing the .MAI contents as they will now be "absent", but you can see the contents directly using any editor. You can then search for the offending spam emails and delete them at your leasure without handcuffing the system (delete in reverse order). Once this is done, you can rename them back to the original directory and "cleanup" the mail system over a weekend. This should clear out the references in the MAIL.MAI file that pointed to the now deleted files.
