Could be a bad system.   Bad or down-revision or mismatched firmware.   
Old Integrity systems are a problem without a support contract, as the 
firmware has largely disappeared behind a paywall.

As for EFI, boot the DVD, maybe verify the entries, but it's typical to 
remove the existing boot alias entries related to the boot disk, and to 
re-add them using the BOOT_OPTIONS.COM tool.

Try enabling the boot-time diagnostics.   See the primary boot flags

But given you are working with hardware that has apparently already 
failed once and could well have deeper issues with the configuration or 
firmware or support, who knows what's wrong with this box?
