Try an Ultrium cleaning cartridge.  Try the cartridge in a different 
Ultrium IV drive, too.

What was the BACKUP command used to create this saveset?

If you've just initialized the target, re-initialize the target with 
appropriate /MAXIMUM_FILES and /HEADERS settings specified.  The 
defaults for those have been poor choices.  Though those cases don't 
usually toss VOLINV errors.
