There's a RewriteRule in your future, or Apache mod_speling, or some 
remediation with awk or otherwise, or similar.

The doc has had these issues forever, and was only originally intended 
for OpenVMS and Windows, though macOS will also work.

I'd be surprised if VSI would expend effort fixing this, and would 
simply continue the case-insensitive requirements.   Though I'd hope 
that the VSI replacement for Document can or would involve the use of 
tidy-html5, LinkChecker or similar on the HTML output.  (Tidy can be 
built with FILENAMES_CASE_SENSITIVE based on a quick look, and it's 
fully callable application code.)  I'd be surprised if there weren't 
other lint and link-checking tools for this purpose, too.  But for now 
and likely for the next decade, we get to aim awk at this, or use 
Windows, macOS or OpenVMS to access the files, or to serve the files 
from a case-insensitive or case-fixing server.
