At least 3 different possible editors may have been used, default
appears to be eve with an eve keypad layout that does not match the
labeling of any VT-xxx keyboard that I have ever seen. EVE has multiple
optional personalities.
I use EVE with the EDT keypad and a bunch of custom settings.
The backspace character is Control-h. Putty will also by default send
it when you press "shift-backspace".
The behavior of an editor to the backspace code is dependent on the editor.
Your pictures show that you have the default settings.
I would not put a /perm a set term command in a login.com. And /perm
will not save what /inquire discovers.
I do not have a serial connection with Putty and I can not reproduce
your issue on VMS 8.4.
The last time I tried to use putty on a serial port was at least a
decade ago and putty crashed with a segfault on every attempt to do so.

I have not seen any issues with VT escape handling in Putty with editing
files or other VMS utilities.
Since set term/inquire seems to have fixed the issue, I suspect that the
terminal size/type set on VMS was different than what was set in Putty.
