1) Could also include logging in from a dedicated device, using
certificates, passwords for the device, passwords for the login,
generating codes with some hardware or software token---not OR but AND.

2) might also involve some additional hoops.

Also, at some stage, some sort of monitoring (keylogging, video capture) 
might be involved.

Some folks have to log in through half a dozen layers of stuff.
