As discussed, this is down to the 5th parameter in the call to FORMS$ENABLE which needs to specify the form_specification as well as using FORMS$AR_FORM_TABLE as the first parameter, if you're using a shareable.
It _is_ mentioned in the Programmers' Reference, but it's very small print and the examples don't appear to mention it at all, I have a feeling that they only run because they are using .FORM files, rather than using the shareables which are created.

I will be putting something in the VSI Knowledge Base on this, in case anyone else is interested.
