Dan, Can you take a picture of this device ?  I'm looking to cannibalize MMJ jacks for a project.  I found an H3107-C device, which is a PC board with 16 connectors facing up that connects to a 36 pin centronics connector. When you say "punch block" to RJ45, do the RJ ports have the tab to the right side instead of the center ?
