consider to use the DKLOG SDA extension to find out the details about the IOs done during the INIT command:

$ ANALYZE/SYSTEM
SDA> DKLOG START $1$DGAx:
SDA> SPAWN INIT $1$DGA1: TEST
SDA> DKLOG STOP $1$DGAx:
SDA> DKLOG SHOW $1$DGAx:
...

Maybe compare that output with a working INIT command on some other disk.
