the process is busy waiting for an IO to finish on file HOPAK$DNA2:(288,1,0) <- this is the file ID.

SDA> SHOW DEV DNA2: should list the current IRP address

SDA> READ SYSDEF
SDA> FORMAT <irp address>

Is the console terminal 'broadcast enabled' ? The default for DECwindows may be to disable broadcasts. Any Mount-Verification messages in OPERATOR.LOG ?

You may consider to start IO tracing ( see http://de.openvms.org/Spring2009/05-sda_extensions.html#IO ) in the runn9ing system, wait for the next hang, force a dump and then look at the IO trace in the dump...
