Going even further off-topic from VMS;   and maybe an understandable 
situation:

A co-worker got his Master's Degree in Computer Science (in the early 
days of such), and then left to work for the correctional system.
His entire job centered around the database of all criminals.

One day an inmate on weekend visitation or parole (or some such) did
something; I forget whether he died, or killed somebody, or whatever.
In any case, it created a new category, and the whole database had to
be reconstructed to handle the very improbable case that it could happen
to someone else in the future.
