Yeah, and its usage got generalized in HTML4, and in the earliest 
Firefox browser versions, and in other tools.  This stuff is old...

You're dealing with an enterprise site and its legions of locked-down 
client devices.

When it arrives, BYOD tends to blow open a hole below the web waterline 
in those organizations.

BYOD: Bring Your Own Device; an anathema to centralized single-browser 
IT groups everywhere, and particularly those that have heavily invested 
in custom applications tied to specific browser features, and then (and 
for various possible reasons) haven't maintained those applications and 
tools.

Different browsers and different versions do different things with the favicon.

And again, this favicon stuff is old...  Even older than the VMS Apache 
version. :-)
