After trying several other things, here are some additional problems
I ran into when I attempted a foreign boot:

(boot dka600.6.0.6.0 -flags 0,80000)

%IOC$INIT_SBD-I-FORBOOTREQ, Boot flags requested foreign boot.
A foreign boot was requested (-flag 0,80000). This
allows key components and configuration information
to be read from a secondary boot media.
The system will now locate the device where the secondary
boot media will be loaded. You will be prompted to mount
the secondary and primary boot media as needed. You may be
prompted multiple times, depending on the device selected for
the secondary media, and the components that are loaded.
The following devices were specified on the boot command:
1 = DKA600
2 = DVA0
On which device will the secondary media be mounted [default 2 (DVA0)]:
Is the secondary boot media ready in drive DVA0 (Y/N) [Y]:
No Floppy Disk In Drive

Where DKA600 is a Yamaha CRW4260 external CD-R/RW drive. I've also
used the DEC RRD43, hoping SCSI 'not ready' errors would go away.
I've also tried various different floppy diskettes. I guess it
wouldn't help if the internal floppy diskette drive in my Multia/UDB
would turn out to be broken/defective...
Here's some information, including about the SRM:
