The former.  There are two provinces, North Holland and South Holland, 
to which this refers, and 12 other provinces.

A closer analogy would be referring to all British people as Welsh.

But the Canadians ARE Americans, as are the Cubans, the Argentinians, 
the Hondurans etc.
