Apparently some of the "optional" fields (like "what do you want to use this for?" are secretly "mandatory" and requests with that left blank get ignored. I don't know if that happens at the form-submission level, somewhere in the electronic-processing level, or after the rep in India looks at the request. Check what you submitted and re-submit if necessary. I had a request processed as recently as mid-March of this year.
