[Info-vax] printing issue form/stock mismatch

hi 

I'm trying to setup a que to use tray 3 but somehow ending up with a form/stock mismatch error.  I'll try to provide all the info let me know what else you would like to see. 

here is the form

what I don't understand in the output below and where I think my problem is, that it has form =DEFAULT  where is that being defined?  or is it something else?
