Given the storage I/O timing, the hard disk and the storage controller 
are the most likely suspects. Then the blade and giblets.
A common pattern for a failing hard disk includes getting slower. This 
failure also usually shows up among the ELV errors. Usually. Hard disks 
are easy to swap, and cheap.
The storage controller would be the next target for swappage.
Pragmatically, swap this whole show over to a spare i2 blade, and then 
rummage the sketchy blade, or send the sketchy blade out for depot 
service.
Y'all will also want to investigate the VSI versions, as there are 
other issues and corruptions known in OpenVMS as far back as you're 
using here, and that gets i4 and i6 support.
