Tab-oriented browsers?   Um, no.  Microsoft IE5 added favicon.ico.  
Back around 1999.  Then it got added to the HTML4 standards.  For some 
yet-newer icon stuff, there's apple-touch-icon and friends for the web 
clip support, and the HTML5 scalable icons that are starting to see 
some use.

Stock icons should be available (somewhere).  (I haven't looked at a 
current "naked" Apache distro recently to see if there are default 
icons included there. FWIW, the VMS Apache V2.2 port is 2.0.63, and 
that's two major releases behind current Apache (2.4.2), so I wouldn't 
assume what source code and icons is included in the VMS port aligns 
with current Apache reality.  Yes, the newest Apache 2.0.x version is 
2.0.64; we might see that one.  But don't get me started on the 
nonsense where the SWS V2.2 kit version isn't actually the Apache 2.2.x 
version.)

Some browsers and some versions of browsers can be a little odd around 
where they expect the favicon.ico file to exist, or you can have 
multiple virtual hosts around, so a RewriteRule or maybe an AliasMatch 
can help with that.

If you're looking to use some newer web-facing bits and maybe run with 
what those pesky tab-using kids on your web-lawn are (also) fond of, 
I'd suggest looking at a web content management system (CMS, and no 
relation to DECset CMS); these CMS packages often include support for a 
favicon and other related pieces as part of the theme.  I won't mention 
the nginx web server.  Nope.  Won't mention it at all.  Nor Tir and 
Mongrel2 and Lua.  Nope.  No comment on Python and Django, either.  
Nope.  Or Drupal.org or the rest of what's in the cmsmatrix.org.  Nope.
